Kindergarten Physicals and Immunizations are due the first day of school in August if your child participated in Kindergarten Screening.
If your child does not receive a health assessment as required, he/she will not be allowed to attend school until a current health assessment is obtained and proof is submitted to the school nurse.
The health assessment definition includes: health history, physical examination and other screening tests as medically indicated. The examiner determines what lab and other screening tests should be done. However, when the assessment is provided to a Medicaid participant child, Medicaid requires that certain tests be done as part of the Kan-BE-Healthy Program.
Qualified examiners include physicians, other staff working under direct supervision of physicians, or nurses certified by the KDHE.
The local county health departments perform excellent health assessments. If you choose to obtain the assessment from the county health department, please call one of the numbers below as soon as possible to schedule the assessment. (Please do not wait until next August to schedule a physical with the health departments, as they are usually booked up by that time).
Health assessment forms and proof of immunizations should be forwarded to the school nurse by the parent or legal guardian after the assessment is completed.
If your child has already had a complete health assessment (since August 2006), please submit proof of the assessment to the school nurse by the first day of school.
Your Health Care Provider can tell you if your child's immunizations are up-to-date. Required immunizations include DTP, IPV, Varicella (chickenpox disease or vaccination), Hepatitis B and MMR.
